Abstract

Older adults are at high risk of falls due to declining physical function and balance caused by intrinsic and extrinsic factors. Unsafe environmental conditions are among the major contributors to fall incidents. This study aimed to describe environmental modification strategies to minimize fall risk among older adults at UPT PSTW Jember. A descriptive case study was conducted on a 77-year-old female older adult with a high risk of falling. Data were collected through observation using the Home Safety Self-Assessment Tool (HSSAT), interviews, documentation, and field notes during four visits. Following environmental modifications, including the installation of handrails, use of anti-slip mats, facility arrangement, and safety education, the HSSAT score increased from 6 to 9. The participant also reported feeling safer during daily activities. Simple environmental modifications are effective in reducing fall risk among older adults and can be applied as a fall prevention strategy in nursing homes and home settings.
